About The Position

The RN Weekend Case Manager delivers comprehensive case management services for patients and families during weekend hours. This role supports safe, efficient discharge planning, continuity of care, and coordination across interdisciplinary teams. Requirements

  • 3+ years of experience in one or a combination of the following: case management, care coordination, utilization review, patient intake, discharge planning and troubleshooting fund resources, quality assurance, clinical pathways, continuous quality improvement, or state and federal health plans or commercial insurance plans in a clinical or managed care environment.
  • Demonstrated proficiency in utilizing Microsoft Office tools/applications.
  • Associate’s degree in Nursing is required.
  • Current Registered Nurse License in the State of Texas required.
  • Basic Life Support (BLS) Certification for Health Care Providers required within 30 days of employment and must be maintained per Medical Center Policy, MC 175.
  • Valid Texas Driver’s License required.
