Our client is a home care agency offering Personal Care, Respite Care, In-Home Care, Speech Therapy, and Brain Injury Services. The Case Manager RN provides skilled nursing care per the client's plan of treatment, including comprehensive health and psychosocial evaluation, monitoring of services, teaching and training activities, and provision of direct nursing care. Works under the supervision of Director of Clinical/Professional Services or designee. Exercises supervision of assigned personnel including licensed practical nurses, home health aides, and other support staff. Case Managers are expected to be available by telephone and in person to client/staff Monday-Friday 8a.m.-5p.m. If census is low, then Case Manager will work in office. May require inclusion in on call rotation, M - F after 5 pm, and weekends depending on office location. Full time, salaried Registered Nurse Case Manager will be expected to complete visits on a point system. Point system as follows: Start of Care= 2 points, Resumption of Care= 1.5 points, Discharge= 1.5 points, Recertification= 1.5 points, Skilled Nurse visit=1 point, Transfer= 0.5 points, HHA supervisory visit= 0.5 points, brief visit= 0.5 points. Case Managers will be required to complete a minimum number of visits each week to equal 25-30 points. If the number of visits/points continually fall below this amount the Case Manager will be changed to a per visit rate. Any visits greater than 30 points per week will be compensated at a per visit rate.
