About The Position

Position Highlights: Job Type: Full-Time Location: Community Hospital/Western Orthopedics & Sports Medicine/Spine & Pain Schedule: Full-Time Application Deadline: Open until March 6, 2026 Responsibilities: Coordinates the continuity of patient care with assigned patients and families following hospital admission, discharge, and ER visits. Supports patient self-management of disease and behavior modification interventions. Provide triage to joint and spine patients, following nursing guidelines, protocols, and physician direction. Ensures Western Orthopedics & Sports Medicine and Grand Valley Spine meet the standards of The Joint Commission (TJC). Directs preparations and ongoing monitoring of accreditation standards. Maintains and monitors quality matrix and files in compliance with TJC. Incorporate factors related to safety, effectiveness, cost, and impact on practice in planning and delivering patient care in regard to resource utilization. Maintains data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Center of Excellence for Orthopedic and Spine patients as directed by The Joint Commission. Evaluates clinical care, utilization of resources, and development of new clinical tools, forms and procedures related to The Center of Excellence. Requirements: Bachelor's degree in nursing At least three (3) years of related experience in case management and/or utilization management, or similar experience required. Experience in orthopedics or spine preferred. Certification in Case Management preferred or willing to obtain within three (3) years of employment Registered Nurse (RN) in the State of Colorado . Compensation: $39.00 - $44.85 per hour, depending on education and experience. Bonus: $5,000 Discretionary bonuses, relocation expenses, merit increase, market adjustments, recognition bonuses, and other forms of discretionary compensation may be available. Requirements

  • Bachelor's degree in nursing
  • At least three (3) years of related experience in case management and/or utilization management, or similar experience required.
