Requirements

Current RN license in Colorado (BSN preferred)
Minimum 1 year of nursing experience (home health or acute care preferred)
Strong assessment, clinical, and communication skills
Ability to work independently while being part of a team
Compassion, adaptability, and a true passion for patient-centered care

Responsibilities

Manage and coordinate the care of patients in their homes
Perform initial assessments, recertifications, develop care plans, and provide skilled nursing care
Collaborate with physicians, therapists, other community partners, and caregivers to ensure optimal outcomes
Educate patients and families on managing conditions and medications
Document accurately and timely in the EMR
Participate in case conferences and team meetings
SN visits as needed
