RN Case Manager
About The Position
Continuum Care Hospice, an affiliate of Legacy Health Services, is looking to hire an experienced and dedicated RN Hospice Case Manager to work with our team of professionals. The ideal candidate will be a Registered Nurse with at least one (1) year of experience. Hospice experience is a plus. Hospice is not a place, it’s there when needed, where needed, whether it’s a private residence, nursing facility or assisted living. Our professional hospice team works with our residents, their family and physicians to provide medical, psycho-social, emotional and spiritual care. Minimal On-Call and excellent wage. We offer great pay, autonomy and a family of seasoned staff! Summary The RN Case Manager is responsible for the holistic nursing care of the patient, and a member of the interdisciplinary team, provides for the total physical, psycho-social, emotional and spiritual needs of the patient and family.
Requirements
- Two to five years RN experience
- Strong clinical assessment skills required
- Ability to work as part of a team, have passion, empathy and work independently in patient/family’s homes
- Must be a graduate of an accredited school of nursing
- Current nursing license with the State
Nice To Haves
- Hospice experience preferred
- BSN preferred
