RN Case Manager

We are seeking a Motivated RN to join our team in Salt Lake as a Case Manager to assist with Home Health Skilled Nursing visits. Requirements
- Active RN License.
- Current BLS Certification.
- Valid Driver's License and Vehicle Insurance.
- Strong time management and prioritization skills.
- Excellent communication, assessment skills and attention to detail.
- Visits are primarily within the Salt Lake Valley; however, candidates must be willing to travel to Utah County as needed.
Nice To Haves
- Previous Home Health experience preferred.
Responsibilities
- Provide private duty nursing care to patients in their homes to help with caseload growth until full to continue in a supervisory capacity.
- Provide skilled nursing visits to establish census.
- Administer medication, treatments, and therapies as prescribed by the physician.
- Monitor patient's condition and report any changes to the healthcare team.
- Assist with activities of daily living and provide emotional support to patients and their families.
- Collaborate with other healthcare professionals to ensure optimal patient care.
- Maintain accurate and complete medical records using web-based electronic medical records system.
- Assist with marketing and market growth development in Southern Utah and the surrounding areas.
- Partner with talent acquisition and patient alignment.
- Attend careers fairs, partner with local universities and attend various marketing opportunities as needed.
