Requirements
- Current, unrestricted RN compact license
- 3+ years of experience in a hospital, acute care, or direct care setting
- Basic level of experience with Microsoft Office (Word, Excel, Outlook)
- Designated workspace and access to install high-speed internet via hardwire in home
Nice To Haves
- BSN
- Certified Case Manager
- Case Management experience
- Experience or exposure to discharge planning, utilization review, concurrent review or risk management
- Experience in a telephonic role
- A background in managed care
Responsibilities
- Make outbound calls and receive inbound calls to assess members' current health status and gaps in care
- Completes assessments evaluating members for clinical and social needs timely and effectively
- Follows model of care requirements as outlined in policies, procedures, and job aids
- Evaluates members for ongoing needs and completes referrals as appropriate
- Manages, communicates, and updates the plan of care with members
- Collaborates with other programs and departments as necessary
- Collaborates with Primary Care Physicians regarding medication concerns, questions, or other member needs
- Ability to work independently and follow program processes
