Requirements

  • Current, unrestricted RN compact license
  • 3+ years of experience in a hospital, acute care, or direct care setting
  • Basic level of experience with Microsoft Office (Word, Excel, Outlook)
  • Designated workspace and access to install high-speed internet via hardwire in home

Nice To Haves

  • BSN
  • Certified Case Manager
  • Case Management experience
  • Experience or exposure to discharge planning, utilization review, concurrent review or risk management
  • Experience in a telephonic role
  • A background in managed care

Responsibilities

  • Make outbound calls and receive inbound calls to assess members' current health status and gaps in care
  • Completes assessments evaluating members for clinical and social needs timely and effectively
  • Follows model of care requirements as outlined in policies, procedures, and job aids
  • Evaluates members for ongoing needs and completes referrals as appropriate
  • Manages, communicates, and updates the plan of care with members
  • Collaborates with other programs and departments as necessary
  • Collaborates with Primary Care Physicians regarding medication concerns, questions, or other member needs
  • Ability to work independently and follow program processes
