About The Position

The Case Manager provides clinically-based case management to support the delivery of effective and efficient patient care. The role integrates utilization management, care coordination, and transition planning functions. The Case Manager II has the overall accountability for a designated case load and plans effectively in order to meet patients needs, manage the length of stay, and promote efficient utilization of resources. In collaboration with the interdisciplinary team, the case manager II supports the physician in facilitating patient care with the objective of enhancing the quality of patient outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. In this role, you will conduct comprehensive patient and family assessments within 48 hours of admission to identify financial, emotional, physical, social, and functional needs while developing individualized discharge plans and reducing readmission risk. You’ll apply approved clinical criteria to ensure appropriate admission and continued stay status, communicate effectively with physicians regarding medical necessity documentation, and escalate cases as needed. This position plays a key role in educating patients, families, and interdisciplinary teams on post‑acute care options, facilitating care progression during multidisciplinary rounds, and addressing length‑of‑stay goals. You will also collaborate closely with Social Work on complex psychosocial cases, lead care conferences, document all assessments and interventions, and ensure compliance with departmental standards, safety protocols, and productivity expectations. Ongoing professional development, participation in process‑improvement initiatives, and support of organizational goals are essential components of this impactful role. (This differential may be adjusted or discontinued based on business requirements.)

Requirements

  • NM State or Multistate License
  • BLS Requires at Time of Hire
  • Level I Candidates: ADN
  • Level II Candidates: ADN + 5 Years of Case Management experience or BSN
  • Level III Candidates: MSN or BSN + 5 years of Case Management experience
  • Level IV Candidates: MSN + 5 years of experience or BSN + 10 years of experience
