Requirements

  • Must be a registered nurse (RN) licensed in Texas (Required)
  • One year of RN experience (Required)
  • Must live in the approximate area specified in the job posting (Required)
  • Good organizational and time management skills
  • Excellent verbal and written communication skills
  • Strong interpersonal skills
  • Ability to multi-task and accomplish multiple goals simultaneously
  • Professional demeanor and appearance
  • Excellent customer relations skills
  • Meets health requirements of Texas

Responsibilities

  • Initial and ongoing comprehensive assessments of home health patient’s needs, including OASIS assessments at appropriate time points.
  • Develop/initiate/revise the plan of care as necessary and initiate appropriate preventive and rehabilitative nursing procedures.
  • Provide those services and/or treatments requiring substantial and specialized nursing skills. Provide skilled nursing care to patients utilizing infection control measures that protect both the staff and the patient (OSHA)
  • Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service. Monitor assigned cases to ensure compliance with requirements of third party payor.
  • Provide effective communication/counseling to patient/family, team members, and other health care professionals in meeting nursing and other needs.
  • Complete appropriate documentation in a timely manner.
  • Supervise and provide clinical direction to HHA/LVN providing services.
  • Demonstrate commitment, professional growth by participating in in-service programs and competency.
  • Promote Agency philosophy and administrative policies.
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned.
