Optum Home & Community Care, part of the Optum family of businesses, is building an integrated care model to holistically address individuals' physical, mental, and social needs, helping patients access and navigate care anytime and anywhere. As a member of the Optum Care at Home team, this role contributes to an interdisciplinary care environment, assisting patients in navigating the healthcare system and connecting them to key support services. This preventive care aims to help patients stay well at home, adding a layer of support to improve access to care and create a seamless health journey across care settings. Registered Nurses in the Optum at Home program primarily work with patients in their home setting, as part of a care team that includes a Nurse Practitioner, Behavioral Health Advocate, Care Navigator, and other supporting members. They deliver appropriate patient care through in-home visits, telephonic outreach and consultations, and interdisciplinary team activities. The Registered Nurse performs care and counsel for the ill, injured, or infirm, promoting and maintaining health for individuals and groups throughout the life span. They manage health problems and coordinate healthcare for Optum at Home patients in accordance with State and Federal rules and regulations and nursing standards of care, including assessment of health status, development and implementation of a plan of care, and ongoing evaluation of patient status and response to the plan of care. Clinical management is conducted in collaboration with other care team members.
