Your Role on the Team Position Summary: The Case Manager accomplishes patients' care by assessing treatment needs developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches, coordinating and providing care that is safe, timely, effective, efficient, and patient-centered.

Requirements

  • All external candidates must have a Bachelor of Science in Nursing
  • Current California RN license
  • American Heart Association BLS Certification
  • Three to five years of experience in an acute care hospital setting (medical/surgical preferred)
  • Previous one to two years of experience as a Case Manager
  • Must be proficient in typing
  • Ability to write and communicate professionally
  • Must be proficient in computer skills including Microsoft Office
  • Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
  • Must demonstrate customer service skills appropriate to the job
  • Excellent written and verbal communication skills in English
  • Ability to maintain a work pace appropriate to the workload
  • Ability to establish and maintain effective working relationships across the Health System
  • Must be able to provide proof of work authorization

Nice To Haves

  • Broad knowledge of Medicare, Medi-Cal and insurance guidelines
  • Critical care experience (For Resource Case Manager)
  • Knowledge of University and medical center organizations, policies, procedures and forms
