About The Position

The RN Care Manager partners with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure seamless, patient-centered Care Coordination across the Continuum of Care. The RN Case Manager conducts patient evaluations, develops and implements transition of care plans, and ensures readiness for discharge. The RN Case Manager is committed to departmental and system-wide goals, policies, and compliance. The RN Care Manager delivers high-quality care and fosters a respectful, collaborative, and service-focused environment.

Requirements

• Registered Nurse (RN) with acute hospital nursing experience
• Associate Degree in Nursing (ADN)
• Prior hospital experience in Care/Utilization Management in an Acute Hospital Setting

Nice To Haves

• Bachelor's or Master’s in Nursing (BSN/MSN)
• Certification in Case Management (CCM/ACM)

Responsibilities

• Monitors resources, escalating clinical needs, and promoting efficient, cost-effective care
• Drives optimal patient flow, enhances continuity, reduces readmissions, manages length of stay
• Supports patient safety and satisfaction
• Daily engagement in multidisciplinary rounds fosters collaboration and alignment of care goals
• Core competencies encompass discharge planning, medical necessity review, and transitions of care
• Intervenes to remove barriers to timely care and reimbursement, while educating staff on CMS Conditions of Participation, and effective Care Coordination
• Ensure patients receive the right level of support after discharge
