RN Case Manager, Eddy Senior Care / PACE - Latham
About The Position
Eddy SeniorCare has a rewarding opportunity for an RN Case Manager to work out of our 385 Watervliet Shaker Road, Latham location! Eddy SeniorCare PACE is a Program of All-Inclusive Care for the Elderly, dedicated to helping seniors remain in their homes and communities while receiving comprehensive health and social services. We are seeking a compassionate and experienced RN Case Manager to join our interdisciplinary team in a Part-Time. This role focuses on providing community-based home visits and coordinating care for PACE participants to ensure optimal health outcomes and quality of life.
Requirements
- Current NYS RN license in good standing.
- Minimum 1 year of nursing experience (homecare or community health preferred).
- Strong communication and organizational skills.
- Ability to work independently and travel within the service area.
Responsibilities
- Conduct in-home assessments and develop individualized care plans.
- Collaborate with physicians, social workers, and other team members to manage participant needs.
- Provide education and support to participants and caregivers
- Monitor clinical status and adjust care plans as needed.
- Document all care activities in compliance with regulatory and organizational standards.
