The primary purpose of this job position is to coordinate and supervise nursing services provided to participants in the PACE Organization (PO). Services may be delivered in the day center or in the participant’s home. The role involves participating as a member of the interdisciplinary team (IDT) and in participant care planning, as well as contributing to Quality Improvement (QAPI), budget planning, and survey preparedness. The position also entails supervision of center and/or in-home staff, including orientation, competency assessment, training, development, and performance appraisal. Key responsibilities include facilitating the integration of new participants into the health care delivery system, ensuring adherence to all federal and state regulations, and reviewing participant medical records for timely and accurate staff documentation. The RN Case Manager will communicate, counsel, and guide participants and families regarding service needs and concerns, making referrals as appropriate. Participation in the administrative on-call rotation and communication with weekend and after-hours on-call staff are also required. The role actively serves on various committees that support daily operations and participates in the preparation and presentation of mandatory education programs. This is a full-time, day shift position, which may require a rotating schedule of every other weekend as the program grows, and administrative on-call approximately every 6 weeks.
