About The Position

MercyOne – Rural Network Position Purpose: The Critical Access Acute Case Manager RN plays a vital role in strengthening care delivery across MercyOne’s critical access hospitals. This position champions consistent, high‑value care management practices that improve patient outcomes, support safe and timely transitions, and optimize the use of clinical resources. Serving as a strategic partner to rural hospital leaders, this role promotes seamless care coordination, strengthens rural–urban integration, and ensures alignment with MercyOne and Trinity Health quality, regulatory, and compliance standards. Through consultation, collaboration, and education, the Case Manager RN advances high‑quality acute and swing bed care across the rural network—helping patients receive the right care, in the right place, at the right time.

Requirements

  • Bachelor’s degree in Nursing required; Master’s degree in Nursing or related field preferred.
  • Active Iowa Registered Nurse (RN) license required.
  • Five (5) or more years of leadership experience in care coordination or case management required; rural healthcare experience strongly preferred.
  • Demonstrated strength in project management, data analysis, problem‑solving, and cross‑functional collaboration.
  • Proficiency with electronic health record (EHR) systems and advanced working knowledge of Microsoft Office applications (Word, Excel, Outlook, PowerPoint, Access, SharePoint).
  • Comfortable operating within a shared leadership and collaborative environment.
  • Demonstrates integrity, professionalism, and a caring presence, with the ability to influence, inspire, and motivate others in alignment with Trinity Health and MercyOne values.

Nice To Haves

  • Master’s degree in Nursing or related field preferred.
  • rural healthcare experience strongly preferred.

Responsibilities

  • Integrates and demonstrates the Trinity Health and MercyOne Mission, Vision, and Values in day‑to-day practice and professional decision‑making.
  • Serves as a clinical and operational resource to rural hospitals, providing expertise in care management, utilization review, discharge planning, and complex care coordination.
  • Supports the implementation of strategic initiatives, standardized workflows, and evidence‑based practices to promote consistency and high‑quality outcomes across the rural network.
  • Partners with rural site leadership to monitor key performance indicators, evaluate trends, and identify opportunities to improve patient care, throughput, and resource utilization.
  • Leads and supports process improvement efforts that enhance care coordination, patient flow, and timely transitions of care in critical access settings.
  • Ensures ongoing compliance with regulatory requirements, accreditation standards, and organizational care management policies across all rural facilities.
  • Acts as a liaison between rural sites, MercyOne leadership, urban hospitals, and community partners to strengthen communication and integrated care delivery.
  • Provides education, coaching, and skill development for rural care management teams to promote consistent practice and high‑quality outcomes.
  • Collaborates with urban hospitals to optimize patient placement, facilitate effective transfers, and support appropriate levels of care throughout the system.
  • Promotes effective swing bed utilization, monitoring performance to support appropriate use of rural resources and overall system optimization.
  • Maintains a working knowledge of applicable federal, state, and local laws, Trinity Health’s Organizational Integrity Program, and MercyOne policies to ensure ethical, compliant practice.
  • Performs additional duties as assigned to support organizational and departmental goals.
