RN Case Manager
About The Position
Complete Senior Care (CSC) is a non-profit Program of All-Inclusive Care for the Elderly (PACE) model of care that encompasses an interdisciplinary team of patient management and clinical leaders. PACE provides a continuum of care that includes primary medical and in-home care, PT, OT, RD, SW, transportation, recreational and aide services. Eligible PACE participants are 55 or older and have been certified by their State as needing a nursing home level of care. Performs assigned job responsibilities in accordance with departmental, state, federal and accreditation standards with regards to RN scope of practice requirements. Assesses participant's physical and functional needs, psychosocial status, home safety, family caregiving burden, etc. utilizing professional RN knowledge. Manages participant's plan of care in conjunction with the interdisciplinary team (IDT) - provides appropriate interventions while tracking outcomes and revises plan of care as needed. Ongoing participant management including aide supervision, reassessments and home visits as needed (competency training provided). Monthly, rotating RN on-call assignment with additional compensation.
Requirements
- Registered Nurse license and current registration to practice as a professional nurse in New York State.
- Certified in CPR.
