Landmark Hospital is seeking a Case Manager to join their team. The Case Manager RN is responsible for the overall coordination of the patient's plan of care and the resources utilized during their stay. This involves assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to meet individual health needs throughout the continuum of care. Key duties include providing discharge planning through referrals and multidisciplinary collaboration, acting as a liaison between patients/families and the healthcare team to ensure timely and effective care, and performing various tasks related to care coordination. The role requires creativity and judgment to ensure patients' unique needs are met in the most appropriate, timely, and cost-effective manner. The ideal candidate is highly detail-oriented, goal-oriented, with superior analytic, critical-thinking, and time-management skills. They should possess a positive, empathetic personality, the ability to work effectively within a multidisciplinary team in a complex care environment, excellent communication and interpersonal skills, and consistently display a professional attitude.
