About The Position
Make a Lasting Difference as a Hospice Registered Nurse Case Manager (RNCM)! Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of Tennesse is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. Schedule: Weekends, On Call Territory: Cookeville In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Requirements
- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
- Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
- Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.
