About The Position

Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996, caring for patients with serious and terminal illnesses throughout North Central Texas. Their mission is to provide compassionate care for those living with an illness while supporting those caring for a loved one. The organization is currently recruiting for an RN Case Manager who will meet with patients and families to explain the scope of hospice services, coordinate patient admissions, and manage ongoing plans of care.

Requirements

• Registered Nurse in the state of Texas
• Hold a valid driver’s license and have a dependable means of transportation
• 1 year of nursing experience
• Case Management experience (hospice)
• Proficient skills using a computer are required

Nice To Haves

• 2+ years of experience in hospice case management strongly preferred
• Microsoft Office suite experience is preferred

Responsibilities

• Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
• Case manage total care of the patient by coordinating with team members/physicians, supervising CNA activities, and participating in staff and Interdisciplinary Team meetings.
• Complete daily paperwork and charting, thoroughly and accurately, according to Policy.
• Participate in on-call rotation.
• May assist with admissions
