About The Position

The RN Case Manager is responsible for the necessary functions of ensuring a successful move-in of all future residents. Here are a few of the daily responsibilities of a RN Case Manager:           Works in coordination with MDS Coordinator Occasionally Performs resident evaluations as required by state regulations and/or Community policy. Assists and guides residents and families with the move-in process and orients them to the community, including reviewing the resident handbook. Assists residents and families with accessing services within the Community and outside the Community. Discharge planning and assistance with the move-out process. Participates in the development and implementation of each resident's individual service plan. Documents case management notes in the resident file as necessary. Communicates with members of the management team to ensure coordination of services to the residents. Oversees and coordinates the Individualized Service Plan (ISP) and the ALR personal data and resident evaluation form. Performs resident evaluations as required by state regulations and/or Community policy. Supports Community guidelines and state regulations for admission and retention of residents.

Requirements

  • RN currently licensed by the State of Florida in good standing.
  • At least 6 months nursing experience. Geriatric involvement preferable.
  • Can generate a warm, friendly, and caring manner on first impression.
  • Can communicate effectively with residents, families, staff, and visitors.
  • Maintain the highest level of confidentiality.
  • Can pass a pre-employment drug screen and criminal background check.
