Schedule: Full Time About Us – Social Services The Social Services team provides discharge planning to every patient who needs assistance with placements, equipment, services, social issues, etc. Each inpatient department is assigned a Social Worker. This includes Freeman West, East and Neosho Our team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possible What You’ll Do Responsible for assessing patients’ medical and psychosocial needs upon admission and coordinating all aspects of discharge planning. Facilitates post‑acute care services, including DME, home health, and referrals to SNF, IRF, and LTACH facilities. Collaborates daily with the care team, patients, and families to ensure timely, appropriate discharge plans. Interpret clinical data to guide care decisions, assess medical necessity, and meet payer requirements. Provides education on disease management, medications, and available community resources to support successful transitions of care.

Requirements

  • Current Missouri Registered Nurse license or current Registered Nurse license from a compact state. If a compact license is held, it must be in the nurse’s state of residence.
  • AHA BLS for Healthcare Providers within 30 days.
  • Clinic Competency Assessment annually by transcript due date.
