RN Case Manager – Pediatric Home Health In person position Our team is growing and we are looking for someone to join our team! Our mid-size home health care company located in Minnetonka, Minnesota is seeking an experienced, compassionate, organized, dedicated and team-approach focused RN clinical manager for immediate, permanent placement. We are a licensed home health care agency providing extended hour nursing services for children and young adults in the Twin Cities, St Cloud and select areas of rural Minnesota. The Clinical Nurse Manager is responsible for coordination of the highest level of patient care. Supervising and providing leadership to field staff nurses in the patient homes. Requirements

Home Care Nursing Management Preferred.
Pediatric Nursing Preferred
BSN/BAN preferred; Associate Degree in Nursing may be accepted with Governing Body approval
Current valid Registered Nurse license from the State of Minnesota
Active Driver’s License in good standing

Responsibilities

Visit our pediatric and young adult clients and conduct assessments
Develop, implement and maintain the care plan
Supervise nursing staff in the field
Facilitate new hire orientation for new nurses
Participate in the training of new nurses and in clinical support
Follows clinical policies and procedures to meet patient and employee needs
Recommends updates and improvements to policies and procedures to meet regulations and remain current
Coordinate care with members of the interdisciplinary team including providers, DME, clinics and hospital staff
Collaborating with other case managers in the office to provide all patients excellent care coordination
