About The Position

As a Home Care RN with UnityPoint at Home, you’ll provide one-on-one care to patients with total autonomy over your role. With a flexible schedule and independence, our home care nurses are in charge of everything from patient admission to the care plan to discharge and make it possible for patients to receive care where they’re most comfortable – in their homes. The RN Case Manager uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The RN Case Manager develops and oversees the home health plan of care under the direction of the physician and ensures appropriate interdisciplinary involvement to ensure the best outcome for each patient. The RN Case Manager delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.

Requirements

  • Previous cardiac and wound management experience preferred.
  • One year of nursing experience caring for a similar patient population.
  • Basic knowledge of current, frequently used medications, including modes of administration, actions, and side effects.
  • Graduate of State Board approved program for Registered Nurses and valid RN license in state(s) where providing care.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse certification.
  • Completion of Person Centered Care (PCC) course within first 12 months of hire and annual competency validation.
  • Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with re-certification.
  • Competency in a variety of nursing skills and ability to perform without supervision.

Responsibilities

  • Provide one-on-one care to patients in their homes.
  • Manage patient admission, care plan, and discharge processes.
  • Develop and oversee the home health plan of care under physician direction.
  • Ensure appropriate interdisciplinary involvement for optimal patient outcomes.
  • Deliver patient care as directed by the physician in accordance with clinical best practices.
