The RN/Case Manager will be able to demonstrate the following essential competencies, duties, and responsibilities as a result of education, experience and Advanced Wound Clinic initial orientation and ongoing corporate learning. Provides patient care to Wound Care Center patients and handles patient intake and discharge processes as assigned and according to the hospital and nursing standards. May also serve as Case Manager to a group of patients as assigned. This entails: Performing the initial nursing assessment, communicating findings with the assigned physicians, assessing the wound stage and measurements, documenting care plans, evaluating the patients' status in regard to desired outcomes, and identifying patient care situations that require intervention. Participating in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care. Performs Hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics hyperbaric training course and certification. Requirements

Current licensure in the State of South Carolina as a Registered Nurse or Multistate Compact Registered Nurse License
American Heart Association BLS or American Red Cross BLS - Healthcare provider required
ACLS certification within 90 days of hire.
Associate Degree in Nursing required; Bachelors Degree in Nursing preferred.
Minimum of two years' current experience in medical-surgical acute care, with preference given for experience in Wound Care.

Nice To Haves

Wound Care Certification or Wound Care/Home Health experience preferred.
Utilization Review, continuity of care, discharge planning, and performance improvement knowledge is preferred.
