Requirements

  • Registered Nurse (RN) with an Associate Degree.
  • One year experience in case management and/or supervisory/management position involving case management/utilization review/responsibility in an acute care hospital and/or managed care setting required.
  • Current State of Florida license for Registered Nurse or appropriate compact licensure. If compact license held, active FL RN license required within 90 days of hire.

Responsibilities

  • Provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
  • Coordinate the integration of the social service function into patient care.
  • Coordinate the hospital activities concerned with case management and discharge planning.
  • Adhere to departmental goals, objectives, standards of performance, and policies and procedures.
  • Ensure compliance with quality patient care and regulatory compliance.
  • Ensures compliance with all regulatory agency requirements.
  • Maintains working knowledge of requirements of payers, current knowledge of community resources and provider agencies as well as Federal/State and County guidelines.
  • Reviews all admissions regardless of pay source within (1) working day and at least every 72 hours as indicated by patient's condition and insurance company request and documents in the UM module.
  • Performs initial screening of identified patients for appropriate level of care, priority, payer status and high-risk criteria.
  • Provides current clinical data to Health Information and updates as indicated to facilitate correct DRG assignment on a concurrent basis.
  • Identifies appropriate delays in the health care process and documents in the UM module.
  • Demonstrates current knowledge of Interqual and Millimen Criteria for admission, continued stay and discharge.
  • Ensures staff abides by government regulations in transferring acutely ill patients to higher or lower level of care.
  • Participates in and demonstrates current knowledge of assigned patients' condition/readiness for discharge and discharge plan during Interdisciplinary Care Conferences.
  • Adheres to established policy and procedure and standards of care: monitors timeliness of services provided; intervenes as needed to minimize delays.
  • Demonstrates skills and accuracy in collecting and inputting data into hospital computer system as relevant to position.
  • Knowledge of procedures for maintaining security, confidentiality and integrity of patient's medical information.
  • Demonstrates ability to collect, analyze and present data as appropriate for position.
  • Initiates and/or participates in PI process to address and resolve issues impacting quality.
  • Consistently exercises discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
  • Escalates unresolved status, level of care and standards of care issues through the Physician Advisor/established chain of command in a timely manner.
