This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. At least 1 year of Case Management experience and home health agency (or equivalent) experience are required. Must have experience with OASIS and NON-OASIS assessments. 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus. SERVICE AREA: Brown County + surrounding areas in Southern Ohio Primary service area is centralized in Brown County, with typical travel distances in a 10-60 minute radius, with occasional travel up to 90 minutes also required. Option for travel up to or beyond a 2-hour radius is rare, or optional frequently if additional hours are desired, which may include Clermont, Hamilton, Clinton, Highland, Adams, and other counties.
