The RN Case Manager at Moms2B Program functions as the Nurse Educator, responsible for developing and teaching educational programs focused on maternal and infant health within the Department of Obstetrics and Gynecology. This multi-site program aims to educate, support, and nurture women throughout their pregnancy and during their babies' first year of life, with the goal of reducing infant mortality rates in Ohio. The RN Case Manager serves as the lead clinical educational specialist, determining and developing educational programs in both prenatal and postnatal maternal care, as well as conducting childbirth education and counseling sessions for participants at various locations. In this role, the RN Case Manager will develop, establish, and oversee the educational curriculum tailored for healthy pregnancies, specifically targeting various patient populations. They will plan, develop, and implement personalized education programs based on comprehensive needs assessments of program participants. Additionally, the RN Case Manager will review and evaluate the healthy pregnancy program curriculum, ensuring it remains current with new or revised trends in prenatal and postnatal care. Participation in data research activities is also a key component of this position, where the RN will provide summaries of training sessions conducted and metrics on maternal and neonatal mortality rates. The RN Care Coordinator acts as an advocate for patients, ensuring access to appropriate care and services while promoting self-care management and prevention. They will navigate the healthcare continuum by identifying and removing barriers to care and coordinating patient services through the nursing process. Collaboration with a multidisciplinary team is essential to ensure a comprehensive plan of care that aligns with nationally recognized evidence-based standards. The RN Care Coordinator will also educate patients and their caregivers on understanding and managing their diagnoses, treatment, and symptoms, leveraging their expertise in care management principles and community resources to meet individual patient needs. This role includes managing a sub-population of patients to ensure the effective and efficient use of healthcare resources to meet their needs.
