  • Accountable for timely coordination of quality health care services to meet the patient's health care needs across the continuum of care.
  • Initiates and implements utilization/resource management.
  • Continued stay monitoring and discharge planning.
  • Supports optimal patient care outcomes along the continuum of care.
  • Working knowledge of Intequal IS-SI criteria.
  • Working knowledge of Medicare, Medi-Cal, NCQA, Heidis, and other criteria identified by the Quality/Case Management Department.
  • Minimum of 1 year discharge planning experience.
  • Working knowledge of managed care and capitation operations.
  • Valid, current California Nursing License.
  • Excellent written and verbal communication skills in English.
  • Ability to maintain a work pace appropriate to the workload.
  • Ability to establish and maintain effective working relationships across the Health System.
  • Certified Case Manager Certification.
  • Knowledge of University and medical center organizations, policies, procedures and forms.
