About The Position

Responsible for progressing care of patients and ensuring progression of care by reviewing the case promptly and applying IP IQ criteria. Adheres to HCA standards regarding observation management. In addition, the RN SM Lead will be responsible for adhering to the 2 Midnight Process. Works closely with the physician by monitoring the case and will notify the physician of findings, so appropriate and timely decisions can be made to admit or discharge the patient. Identify barriers that prevent progression of care, and either remove the barrier or escalate the barrier to the facility Director of Case Management. Work closely with the case management discharge planners by communicating the progression of care and any barriers to discharge. Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. Responsible for ensuring other RN CM in department are using IQ criteria appropriately. Conduct reviews with the UR physician to adhere to the self-denial process.

Requirements

  • (RN) Registered Nurse
  • Registered Nurse Diploma, Associate Degree, or Bachelor's Degree
  • 2+ years experience in case management OR 3+ years experience in clinical nursing
