At St. Croix Hospice we guide patients and families through the end-of-life journey. Through compassionate care, we focus on our patient's quality of life, empowering them to make the most of their time with dignity, comfort and respect. The Registered Nurse Case Manager (RNCM) is experienced in nursing, and leads each patient's hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff. The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.
