About The Position

Position Overview:
The RN Case Manager (CM) works with patients and families along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care.

Requirements

• Graduate of an accredited school of registered nursing
• Current CA Registered Nurse (RN) License
• Six (6) months of recent full-time acute care nursing experience in an acute care hospital (RN CM I)
• One (1) year of case management experience (RN CM II)

Nice To Haves

• Bachelors of Science in Nursing (BSN)
• Bilingual in Spanish
• One (1) year of experience in Case Management and strong discharge planning skills
• One (1) year of experience with InterQual
• One (1) year of experience with Allscripts and Meditech
