About The Position
This is an out-patient position for an independent and experienced Registered Nurse (RN) Case Manager. The RN-Case Manager is a key member of the interdisciplinary care team, responsible for admitting and coordinating appropriate care for each patient and family.
Requirements
- Licensed RN in the State of Texas
- Effective written and verbal communication
- Ability to travel
- Reliable transportation
- Clean background
- Clean drug screen
- Clean driving record
Nice To Haves
- 1+ years' experience as a RN
- Experience with Hospice or Home Health
Responsibilities
- Admitting patients
- Coordinating appropriate care for each patient and family
- Performing assessments
- Handling discharges
- Performing pronouncements
