Grace Hospice is hiring an experienced hospice Registered Nurse (RN) in the role of a RN Case Manager Hospice. Your skills, compassion, and commitment will be valued in our supportive, mission-driven organization. The ideal RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life care. In this role, you will collaboratively works alongside an interdisciplinary team to provide end-of-life care to those needing hospice services in their home or community setting. You will provide individualized case management, admission facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with chronic and end-of-life care.

  • Succinctly convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility
  • Provide direct patient care to terminally ill patients; assess, identify and address their individual physical, psychosocial, safety, and comfort needs
  • Anticipate, prevent and treat undesirable symptoms or secondary symptoms
  • Develop, maintain, and manage an individualized care plan that captures the evolving needs of the terminally ill patient
  • Train, instruct and support the patient, family and caregiver
  • Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes from interventions and care provided in a timely manner
  • Consult and collaborate respectfully with the interdisciplinary team and others involved in the patient’s care
  • Prepare and present all needed information for bi-weekly IDG meetings
  • Graduate of accredited school of nursing
  • Current state of Minnesota RN licensure
  • Three years recent nursing experience in acute, long-term care or home care/hospice/public health
  • Patient care planning and patient teaching skills
  • Critical thinker with excellent written and verbal communication skills
  • Current Minnesota driver’s license, insurance and daily access to reliable vehicle
