Traditional Home Health & Hospice Join our team of professionals and make a difference in someone's life. $15,000 Sign-on Bonus for Full-time RN Case Managers As an RN with Traditional Home Health Care, you are able to give each patient your undivided attention and the time they deserve as you care for them. Job Title: Registered Nurse, RN Case Manager Hospice Shift: Full-time, Monday - Friday 8:30a-5:00p Job Location: Carbondale, PA and surrounding areas Our Registered Nurses see patients in the most ideal setting - the patient's home! Our office is located in Dunmore, PA. At Traditional, we do our best to keep each RN's case load within a sensible geographic territory.

Requirements

  • Current license as a Registered Nurse in Pennsylvania
  • Must have current driver's license
  • Ability to travel to case assignments
  • Excellent assessment skills, documentation skills and ability to meet deadlines
  • Ability to work independently with minimal supervision and manage time effectively
  • Strong interpersonal skills
  • Demonstrates good verbal and written communication, and organizational skills

Nice To Haves

  • Acute care and/or home health experience is preferred.
  • Having experience with hospice patients or in the hospice environment is a plus.
  • Understanding of Hospice philosophy and principles of death and dying.

Responsibilities

  • Admits patients to Traditional Home Health Care (Hospice).
  • Assesses patient status and develops plan of care in collaboration with physician, nursing supervisor, patient and family members.
  • Administers patient care, management and evaluation of the care plan, teaching of the patient in accordance with physician orders.
  • Reports patient status changes, care, and progress to physician and nursing supervisor on a continuous basis.
  • Revises plan of care by consultation with physician.
  • Timely and accurately prepares, maintains and submits meticulous documentation as required by Traditional Home Health Care including all interactions with patients, case conferences, medication changes, various OASIS time points, and changes to plan of care.
  • Acts as a liaison and maintains contact with patients, therapist, nursing supervisor, and other members of the health care team.
  • Informs patients of other services that are available and makes referrals to other community resources as needed.
