About The Position

The RN Case Manager holds a crucial role in directing and confirming prompt quality care for hospice patients. The RN Case Manager directs, and coordinates activities involved in hospice patient care by performing the following duties personally or through subordinate staff. Ensures regulatory oversight compliance for response, care, documentation, ongoing education/CEU’s, confidentiality, security, and safety. Provides sensitive and thorough education/training to patient’s primary caregiver(s) for proper participation of their care responsibilities identified in POC. Ensures the overall direction of company operations with the highest degree of company expectations, industry standards and legal compliance.

Requirements

  • Graduation from an Accredited RN program; BSN or Advanced Education or certification preferred
  • Maintain current Arizona RN License and any applicable CEU’s
  • Maintain current CPR Certification
  • Strong interpersonal and communication skills (written and oral), including the ability to lead discussions in diverse groups of varying size.
  • Well organized, detail oriented and able to work independently with minimal supervision.
  • Advanced level competency in electronic medical record software.
  • Must possess and maintain; a valid AZ driver’s license, clean driving record, and maintain company insurance qualification of minimum coverage as follows: $100,ooo Per Person Bodily Injury, $300,000 Per Incident and $50,000 Property Damage Personal Insurance Policy.

Responsibilities

  • Executes and enforces compassionate care for patients, families, and our community at the highest level of excellence to uphold the image of Hospice of Havasu.
  • Provide leadership at IDT Meetings for coordination of care, continuous assessment, and the implementation of the interdisciplinary plan of care.
  • Responds promptly and accurately for seamless compliance with Medicare Guidelines and Hospice of Havasu Expectations.
  • Provide directly of through subordinate staff services necessary for the palliation and management of terminal condition including pain and symptom management.
  • Ensures all activities are performed consistently with empathy and consideration in supporting our philosophy that all hospice functions must maintain focus on assisting patients and families during this critical time while maintaining professional boundaries.
  • Supports the implementation of company programs, ongoing training & certifications, regulatory compliance, care enhancements and safety for operational excellence. Provide supervisory functions and evaluation of Visit Nurses, and CNAs including participation in evaluating employee performance.
  • Accountable for ensuring all activities are performed efficiently for medical charting in electronic medical billing software. Following POC guidelines for update, review, patient visits and ensuring all activity is properly recorded and updated throughout services period until discharge. Ongoing evaluation of operating expenses and ensuring controls are in place to achieve business objectives and to avert business losses.
  • Conducts new hire, periodic and as needed training of all subordinate staff to achieve and maintain expectations for safe and fair operational practices.
  • Educate and train primary caregiver(s) to ensure patient specific POC can be followed, ensure all resources available from Hospice of Havasu are understood by patients, family, and caregiver(s).
  • Supports a high-performance culture by reporting any/all violations and or performance concerns immediately, (within 8 hours), to Clinical Services Director.
  • Maintains supply and confirms appropriate PPE use by all POC participants for safe practice compliance.
  • Immediately available by phone throughout shift to respond to calls, incidents, and emergencies.
  • Maintains ongoing relationships and integration of other supporting functional teams of HOH.
  • Ensure compliance with all local, state, and federal laws.
  • Properly uses and maintains equipment.
  • Other duties as assigned.
