Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six months or less, serving individuals in their home, nursing facilities, and in the hospital. Requirements

• Associate’s degree in nursing
• 1 year of relevant experience preferably in Hospice
• Michigan Registered Nurse (RN) license upon hire
• State of Michigan Driver’s license upon hire
• AHA or ARC Basic Life Support (BLS) within 90 days

Responsibilities

• In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's end-of-life care needs.
• Create and continually adjust an outcome-based plan of care through utilization of the nursing process, relationship centered care principles and holistic philosophy.
• Assure the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.
