About The Position

Traditions Health is seeking a dedicated RN Case Manager to join our growing Hospice Team in Glen Allen, VA. This full-time position focuses on providing high-quality nursing care to patients in their homes, coordinating with an interdisciplinary team to ensure the best possible quality of life for patients and their families. Requirements

  • Current State license as a Registered Nurse (RN).
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • One year of experience as a Registered Nurse in a clinical care setting required.
  • Good interpersonal skills and nursing skills as defined by generally accepted standards of practice.
  • Proof of current CPR certification.
  • Reliable transportation and valid driver's license with auto insurance.

Nice To Haves

  • Home health experience preferred.

Responsibilities

  • Administer skilled nursing care for clients of all ages in their place of residence.
  • Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients/clients.
  • Develop, initiate, and revise the plan of care as necessary to ensure quality and continuity of care.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Counsel patients/clients and their families in meeting nursing and related needs.
  • Monitor assigned cases to ensure compliance with requirements of third-party payors.
  • Prepare clinical and progress notes, completing appropriate documentation in a timely manner.
  • Supervise, teach, and provide clinical direction to other nursing personnel.
  • Perform on-call responsibilities and provide on-call services to patients/clients and their families.
