Description: As a Nurse Case Manager (RNCM), your role will be to create, plan and direct the hospice care team utilizing nursing processes of assessment, interventions, implementation and evaluation. You will effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.

Requirements

• Current RN License in the State of Georgia
• 1-2 years nursing experience (preferred)
• 1-2 years hospice experience (other candidates may be considered with experience in skilled nursing, home health, acute care, med surg or ICU)
• Utmost compassion & integrity
• Driver’s license, insurance and vehicle
• Excellent Time Management Skills
