The position involves providing care to assigned patients in a caring, safe, and efficient manner while also providing leadership to staff. The role requires evaluating outcomes based on established clinical, financial, and utilization data, demonstrating knowledge of hospice palliative care, and recognizing different cultural attitudes towards death and dying. The nurse will participate in the creation of the plan of care and ensure its implementation by the interdisciplinary group, while also being responsible for interviewing potential residents and assessing them in compliance with regulations.

Requirements

  • Bachelor's degree in Nursing strongly preferred.
  • Graduate from an accredited nursing program.
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing.
  • Possesses multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and maintains required department specific competencies and certifications.
  • Must possess a valid driver’s license.

Responsibilities

  • Provides care to assigned patients in a caring, safe, and efficient manner.
  • Provides leadership to staff.
  • Evaluates outcomes based on established clinical, financial, and utilization data.
  • Demonstrates knowledge of hospice palliative care and hospice philosophy.
  • Recognizes different cultural attitudes and values towards death and dying.
  • Identifies the five stages of grieving process.
  • Explains advanced directives and clinical requirements.
  • Utilizes independent nursing judgment when integrating health care.
  • Participates in the creation of the plan of care.
  • Ensures the plan of care is implemented by the interdisciplinary group.
  • Interviews potential residents and assesses them in compliance with regulations.
  • Establishes a service plan for each resident and evaluates it on an ongoing basis.
  • Communicates effectively with patients, family members, and the health care team.
  • Demonstrates independence, assertiveness, and critical thinking.
  • Documents clearly and solves problems.
  • Incorporates teaching/learning principles and adapts teaching to age group.
  • Facilitates groups and implements projects when necessary.
  • Establishes priorities and demonstrates strong flexibility and organizational skills.
