About The Position

The Home Care RN/Case Manager is responsible for coordinating and overseeing patient care plans in home health settings. This role involves conducting patient assessments to evaluate health status and needs, collaborating with healthcare providers, patients, and families for optimal care, and monitoring patient progress while adjusting care plans as necessary. The RN will educate patients and caregivers on treatment, medication, and health management, ensure compliance with regulatory standards and documentation requirements, facilitate communication between multidisciplinary teams, and manage referrals and coordinate services with community resources.

Requirements

• Associates Degree in Nursing or BSN, required
• Current RN licensure in state of employment, required
• Two or more years healthcare experience, preferred
• Case Management or related experience, preferred
• Weekend and holiday coverage required

Nice To Haves

• 4 Year / Bachelors Degree, preferred
• Knowledge/Competence of Microsoft office products and keyboarding
