About The Position

The position integrates health care for clients from pre-admission to post discharge, enhancing patient/resident flow and interdisciplinary communication. The role requires independent nursing judgment and knowledge of age-specific care methodologies. You will work with clients across all age groups, coordinating care and facilitating communication among the health care team. The position demands strong organizational skills, flexibility, and the ability to work with minimal direction.

Requirements

• Bachelor's degree in Nursing strongly preferred.
• Graduate from an accredited nursing program.
• Minimum of two years' clinical experience preferred.
• Currently holds an unencumbered RN license with the State Board of Nursing.
• Maintains required department specific competencies and certifications.
• Valid driver's license may be required depending on location.

Responsibilities

• Integrate health care for clients from pre-admission to post discharge.
• Coordinate and sequence the client's care to enhance patient/resident flow.
• Utilize independent nursing judgment in health care integration.
• Communicate effectively with patients/residents, family members, and the health care team.
• Monitor, evaluate, and trend patient/resident responses using structured care methodologies.
• Demonstrate independence, assertiveness, and critical thinking in patient care.
• Document patient care clearly and solve problems effectively.
• Facilitate groups and implement projects as necessary.
