Requirements
- Active RN license
- Graduate of accredited nursing program (BSN preferred)
- BLS required
- Previous home health, hospice, or case management experience preferred
- Knowledge of Medicare/insurance guidelines preferred
- Strong communication, organization, and problem -solving skills
Responsibilities
- Manage patient care in home health and hospice settings
- Perform assessments and develop individualized care plans
- Coordinate services with providers and interdisciplinary teams
- Facilitate transitions of care and discharge planning
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and families
- Ensure appropriate utilization of services and resources
- Document care coordination and visits in the EMR
