About Us - Home Care Freeman Home Care's mission is to improve the health of the communities we serve through contemporary, innovative and quality healthcare solutions in the home care setting Our team of professionals shares common values, vision and purpose We work closely with patients, families and physicians to develop individualized care plans to best meet the needs of each patient Services include: Skilled nursing, skilled therapy, medical social work and home health aide services Schedule: Full Time (40hrs/week) Home Health - Direct Patient Care What You’ll Do Provides an ongoing objective evaluation of the important aspects of patients care through regular review of patient charts, and identification of variations in care to provide data for use in the correction of potential problems and to meet the requirements of all regulatory agencies. Relates effectively with interdepartmental and intradepartmental personnel, medical staff, patients, patients families, and outside agencies, including Medicare, Medicaid, and other regulatory and third party payers.

Requirements

  • AHA BLS for Healthcare Providers within 30 days.
  • Auto liability insurance.
  • Clean driving record.
  • Current driver's license.
  • Current Missouri and Kansas Registered Nurse license or current Registered Nurse license from a compact state. If a compact is held, it must be in the nurse's state of residence.
  • Must be able to successfully pass KDADS criminal record check upon hire.
