About The Position

The RN Case Manager position at MCN focuses on providing case management services primarily to pregnant women and children with special needs, ensuring they receive the necessary resources and support. The role requires a knowledgeable RN/BSN/MSN who can perform home visits and telehealth consultations, while effectively managing a caseload and coordinating care for clients receiving Medicaid benefits.

Requirements

• Valid Texas RN License
• Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN)
• At least two years of experience in case management or related field
• At least two years of experience in an acute care setting
• Valid Texas Driver's License with a good driving record
• Ability to pass a criminal history check

Responsibilities

• Conduct home visits and telehealth consultations for case management services.
• Perform comprehensive assessments and draft service plans for clients.
• Review case progress and determine case closure based on established criteria.
• Provide referral, linkage, and advocacy services to help clients achieve wellness and autonomy.
• Document all clinical services and attempts to contact clients by the next business day.
• Develop measurable objectives and goals in collaboration with clients.
• Follow up with clients who miss appointments and document all attempts to engage them.
