Requirements
- Active New York RN license
- Nurse Compact / Multi-state license (required)
- Minimum 3+ years of diabetic care experience
- Must provide own technology (computer, webcam, microphone, speakers, phone, stable internet)
- Strong communication, critical thinking, and organizational skills
Nice To Haves
- Experience in care coordination or case management (preferred)
Responsibilities
- Provide remote case management for diabetic patients
- Conduct virtual assessments and ongoing monitoring
- Develop, implement, and modify individualized care plans
- Collaborate with multidisciplinary healthcare teams
- Document all patient interactions per facility standards
- Ensure compliance with state regulations and organizational policies
- Maintain confidentiality and secure handling of patient data
