About The Position
The RN Case Manager (C3) position at St. Peter's Health Partners offers a unique opportunity for registered nurses looking to make a significant impact in patient care within a dynamic hospital environment. This full-time role is based at either St. Peter's or Samaritan Hospital, with a focus on collaborative management of patient care progression. The RN Case Manager will work closely with the healthcare team to ensure that patients receive comprehensive care that addresses their needs beyond just the acute care episode. The position requires a commitment to evidence-based practice guidelines and a patient-centered approach, ensuring optimal communication and resource management throughout the continuum of care. In this role, the RN Case Manager will engage with patients and their care teams to manage key outcomes, including disease progression and transitions to appropriate post-acute settings. The RN will serve as a patient advocate, emphasizing the importance of the whole patient experience. Responsibilities also include providing ongoing education to staff, patients, and families, and contributing to a high-quality professional practice environment. The RN Case Manager will be expected to travel between Samaritan Hospital and Albany Memorial Hospitals as needed, ensuring that care is coordinated across multiple facilities.
