We are seeking a Registered Nurse (RN) - Care Team Lead/Charge Nurse to: • Provide front-line nursing leadership with oversight of Home-Based Hospital Care (HBHC) RNs and Mobile Integrated Health Professionals (Paramedics), motivating and mentoring teams delivering acute-level care to patients in their homes through innovative, hospital-at-home models. • Lead and coordinate daily operations by assessing staffing and resource needs, collaborating with clinical and nonclinical partners, and coordinating admissions, transfers, and discharges to ensure safe, efficient, and timely care delivery. • Serve as a clinical and operational expert, applying advanced assessment, critical thinking, and evidence-based practice to support quality outcomes, patient safety, and continuous improvement across the HBHC program. • Partner with the interdisciplinary team—including providers, nurse managers, social work, and ancillary services—to establish care goals, support discharge and post-acute planning, and ensure nurses practice at the top of their license. • Utilize transformational leadership and emotional intelligence to foster accountability, professionalism, and staff development while serving as a role model, clinical resource, and coach for nursing staff and learners. • Support innovative care delivery through digital health technology, interdisciplinary conferencing, and high-quality documentation, advancing the mission and strategic goals of UW Health’s Home-Based Hospital Care program.
