About The Position

Community Care is offering a $8,000 Sign-on Bonus for Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Racine County. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. We coordinate and deliver a full range of supportive services that help more than 13,000 Wisconsin residents live safely, confidently, and with dignity. We are Wisconsin-based with local offices supporting each of the 15 counties we serve. Our dedicated team works with members to develop care plans to meet their health and social needs. Requirements

• Education: Associate degree in nursing required.
• Experience: Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
• Licensure: A current Wisconsin Registered Nurse license in good standing.

Responsibilities

• Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs.
• Conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs.
• Complete all required documentation and charting and assists in the development and maintenance of member records.
