About The Position

As a team member of our Senior Community Care (SCC) product, we work with a team to provide care to patients at home in a nursing home, assisted living for senior housing. Connecting. Growing together. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs).
