About The Position

The Nurse Care Manager coordinates and manages the care of patients with chronic or complex conditions, working collaboratively with physicians, interdisciplinary teams, individual patients, and families to promote positive outcomes and ensure continuity of care. This role performs a comprehensive care continuum process that includes assessment, planning, implementation, coordination, monitoring, and evaluation of the services and options required to support each individual's health needs and goals. Utilizing skilled communication, education, and available resources, the Nurse Care Manager promotes high-quality, cost-effective outcomes. Key responsibilities include coordinating services, addressing barriers to care, and optimizing resource allocation while balancing clinical quality and cost management for an assigned patient population. Please note that a video interview through Microsoft Teams will be required as well as potential onsite interviews and meetings.

Requirements

  • Current Registered Nurse (RN) license in state of practice.
  • Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required).
  • Demonstrated clinical nursing experience, and familiarity with patient-related terminology and processes.
  • Demonstrated experience in care management.

Nice To Haves

  • Certified Case Manager (CCM) or other relevant certification as determined at position level.
  • Two (2) years of experience in nursing, preferably in primary care or ambulatory care settings.
  • One (1) year of experience in care management.
  • Experience in chronic disease management.
  • Knowledge of value-based care models and principles.

Responsibilities

  • Assesses patient needs and develop comprehensive, individualized care plans in accordance with National Clinical Quality Association (NCQA) standards.
  • Establishes collaborative partnerships with patients to help them evaluate patterns in healthcare needs, decision-making, lifestyle choices, and resource utilization that impact their health.
  • Advocates for, educates, and coaches patients, families, and caregivers on treatment options, disease management, medication adherence, community resources, and psychosocial concerns.
  • Supports goal-setting and the development of self-care skills appropriate to the patient’s age and developmental level.
  • Facilitates communication and coordination among members of the healthcare team, actively involving the patient in decision-making to reduce fragmentation of services.
  • Monitors and engages patients across the continuum of care, including facilitating transitions and providing support to prevent readmissions and gaps in care.
  • Manages care for an assigned population or panel of patients.
